Cancellation Policy

We believe you deserve clarity when it comes to your care. Below is an overview of our rates and insurance policies

We are in-network with many, but not all insurance plans. Prior to your initial visit, please do the following:
  1. Contact your insurance company to verify that the provider you will be seeing is in network with your policy.
  2. Provide accurate insurance information to our office prior to receiving services.
  3. Prior to your initial appointment, obtain any necessary referrals from your Primary Care Provider if that is required by your insurance company. Your appointment will be canceled if we do not receive confirmation that the referral is approved. 
  4. Inform our office of any insurance coverage changes prior to receiving services. If you do not have your up-to-date insurance information, we will reschedule your appointment or classify your appointment as self-pay.
CareCredit is accepted for self-pay charges of $200 or more. 
  • A convenient way to access care now and over time
  • No interest if paid in full within 6, 12, 18, or 24 months (terms based on approval)
  • If the balance is not paid in full within the promotional period, interest will be charged retroactively from the original purchase date
  • Subject to credit approval
  • All copays and payments are due at the time services are rendered.
  • If you have a deductible, any portion not covered by insurance will be your responsibility.
  • Patients with outstanding balances must pay their balance in full before being seen for additional services.
  • Cancellations made less than 24 hours before a scheduled intake appointment will incur a $100 fee.
  • Cancellations made less than 24 hours before a scheduled follow up appointment will incur a $50 fee.

Initial Assessment

Office Policies
Appointments must be canceled or rescheduled at least 24 hours in advance to avoid a late cancellation fee.
Cancellations made less than 24 hours before a scheduled intake appointment will incur a $100 fee. Cancellations made less than 24 hours before a scheduled follow-up appointment will incur a $50 fee. After the first missed session, any active prescriptions will be paused until client is seen again by provider. If two consecutive appointments are missed, services will be terminated.

Termination- Repeated instances of no-show appointments which are not made in accordance with our policies will result in termination of services. After the first missed session, you will be issued a written warning. A third missed session will be considered just cause for termination and your chart will be closed with our practice.

Late Arrivals

If late to your appointment, sessions may be shortened to respect the next client's session time. If you are more than 15 minutes late, your appointment may need to be rescheduled or canceled.

Communication

Email or portal messages are preferred for non-urgent questions. We are available for non-urgent questions and appointments only. Typical response time is between 24-48 hours. We do not provide crisis support. If you or a loved one is in need of crisis support, please call 911 or go to your nearest emergency department.

Technology Requirements

All sessions are virtual. Please ensure you have:
  • A private, quiet space
  • A reliable internet connection
  • A device with audio and video

Medication Refills

We are committed to providing you with the best possible care, but please note that we are not a 24-hour facility. Timely requests for medication refills are the patient's responsibility. To ensure appropriate care and respect for both patients and providers, it is important to follow these guidelines:
  • Please allow three (3) business days for processing refills. It is your responsibility to notify our office at least 72 hours prior to running out of medicine.
  • Medication refills will be made only at the time of an office visit or during regular office hours. If you do not have a future appointment scheduled when a refill request is made, then your request will not be processed until you have done so.
  • Refills generally will not be given after office hours, during the weekends, or on holidays, so please schedule your appointments accordingly.
  • Plan ahead: Anticipate your prescription refill needs and make sure you have an adequate supply of medication to last through weekends and holidays. Take into account any upcoming vacations or holidays when healthcare providers may have limited availability.
  • Respect emergency procedures: If you have a genuine emergency or urgent medical need, follow the appropriate emergency procedures in your area. If you run out of medication at a time when our office is closed, please head to your nearest emergency department if your health is at risk.
  • Please do not contact our providers directly after business hours, during weekends, or on holidays for prescription refill requests. Contacting providers directly after business hours, during weekends, or on holidays for non-urgent matters, such as requesting emergency prescription refills. may disrupt their personal time and prevent them from fully recharging and being available for emergencies and critical patient needs.
  • Any messages left on our answering machine will not be checked until the next business day and cannot be used for rapid communication after business hours.

Confidentiality

Your privacy is protected through secure telehealth systems that meet HIPAA requirements. We do not share any of your information without your written consent. The only exception to this is if there is a mandated court order; harm towards self or others; or abuse of children, elders, or those with disabilities.
